Background:The main purpose of this study was to assess the effects of Norvasc (amlodipine) on blood pressure (BP) in hypertensive patients and to explore the effects of Norvasc on hypertensive patients on treatment with amlodipine. Materials and Methods: This was a prospective cohort study. Forty hypertensive patients (55 patients with hypertension, 20 patients with diabetes mellitus, and 20 patients with coronary artery disease) were randomized to receive either Norvasc 5mg, amlodipine (10-50 mg, 1-3 times/week), or placebo. At week 3, a control group (20 patients with no blood pressure changes) was also treated with placebo. Blood pressure was measured before and after treatment with Norvasc 5mg and amlodipine. Systolic and diastolic blood pressure was recorded. All patients underwent laboratory and physical examinations and were followed for a minimum of 7 days. Patients were excluded if they were taking drugs that increase the level of dihydrofurosemide (e.g., amlodipine and thiazide diuretics). Finally, a total of 12 patients with hypertension were included in this prospective cohort study. All patients were treated with Norvasc 5mg and amlodipine. The mean age was 67.4 (range, 18-94) years. The mean systolic blood pressure was 105.2/73.2 mm Hg (p = 0.003) and the mean diastolic blood pressure was 106.6/75.4 mm Hg (p = 0.003). Of the patients with hypertension and diabetes mellitus, hypertension was diagnosed in 6 patients (22.6%) and diabetes mellitus in 3 patients (14.5%) during treatment with Norvasc 5mg. Moreover, hypertension was diagnosed in 2 patients (6.4%) and diabetic nephropathy in 1 patient (1%). At week 3, all patients were still on Norvasc 5mg and amlodipine and blood pressure measurements at the same time were not changed.
Results:A total of 12 patients (22.6%) had hypertension and 14 (23.9%) had diabetes mellitus. At week 3, 7 patients (22.6%) were still on Norvasc 5mg and 7 (22.6%) were still on amlodipine (p < 0.001). In hypertensive patients, Norvasc 5mg was associated with a significantly higher systolic blood pressure (p < 0.001) and diastolic blood pressure (p < 0.001) than amlodipine. Norvasc 5mg was associated with a significantly higher systolic blood pressure than amlodipine (p < 0.001) and did not alter diastolic blood pressure (p < 0.001). Norvasc 5mg was associated with a significantly lower diastolic blood pressure (p < 0.001) and hypertension (p < 0.001). Norvasc 5mg did not alter hypertensive patients’ blood pressure (p < 0.001), nor was it associated with hypertension (p < 0.001). Norvasc 5mg was not associated with diastolic blood pressure (p = 0.09). At the end of treatment, all patients still remained on Norvasc 5mg and amlodipine (p = 0.09).
Conclusions:The results of this study suggest that Norvasc 5mg, amlodipine, and placebo were effective in hypertensive patients treated with amlodipine. In patients with hypertension, Norvasc 5mg was associated with a lower systolic blood pressure than amlodipine.
